Purpose and Aim:
To provide registered nurses with the knowledge and skills to prevent and manage pressure damage.

Learning Outcomes Assessment Criteria

1. Understand how to complete a pressure ulcer risk assessment.
1.1 Describe the anatomy and physiology of healthy skin.
1.2  Identify anatomical sites where pressure damage might occur.
1.3  Explain the changes that occur when damage caused by pressure develops.
1.4  Summarise intrinsic and extrinsic factors which raise the risk of skin breakdown and pressure damage.
1.5  Explain how to conduct pressure ulcer risk assessments.
1.6  Justify the findings of risk assessments to prevent and manage pressure damage.
1.7  Give examples of the impact of pressure ulcers on quality of life.
2. Understand how to prevent pressure damage.
2.1 Evaluate strategies for the prevention of pressure damage using international guidelines.
3. Understand how pressure damage is managed.
3.1 Evaluate strategies for the management of pressure damage using international guidelines.
4. Be able to prevent pressure damage.
4.1 Conduct a minimum of three patient assessments to include:
  • Confirming the individual's identity
  • Supporting the patient's language preference
  • Answering the patient's questions or referring them to other healthcare professionals for clarification
  • Obtaining consent
  • Explaining the procedure
  • Respecting the indivuduals dignity and privacy
  • Maintaining the comfort of the patient throughout the procedure
  • Complying with current policies for infection control, record keeping and data protection, safeguarding, health and safety and manual handling
  • Inspecting the skin
  • Classifying and recording skin damage
  • Identifying the pressure ulceration risk assessment score
  • Recording the risk assessment
  • Ensuring pressure redistributing equipment is sourced to comply with individual's needs
  • Supporting the patient with follow up care and advice
4.2  Produce a minimum of 3 care plans to prevent pressure damage.
5. Be able to manage the care of individuals with pressure damage.
5.1 Manage the care of individuals with pressure damage to include:
  • Complying with current policies for infection control, record keeping and data protection, safeguarding, health and safety and manual handling
  • Investigating root causes of pressure damage
  • Collecting data about the incidence of pressure damage in an area of work
  • Justifying wound management regimes
  • Identifying when and how to assess support from the multidisciplinary team
  • Delegating to and supervising colleagues in the prevention and management of pressure damage
  • Providing information and advice about pressure damage to patients, carers and colleagues
  • Recording findings and actions on SKIN bundle documentation
6. Be able to delegate damage care to others.
6.1 Delegate responsibility for pressure damage care to colleagues to include:
  • Explaining the care plan
  • Establishing lines of support and communication
  • Arranging handover
  • Informing the patient and/or their carers
  • Reviewing outcomes of pressure damage care undertaken by others
